Recently I sat in a talk/CME organized by the hospital.
The case presentation was simple. In order to avoid any confusion I think I’ll just take this simple and in a not so detail way.
The title of the talk was - Where is the baby? (pretty catchy eh).
It happened few months back. A 21 years old girl came into the A&E complaining of lower abdominal pain. She was weak, pale, looked dehydrated and a bit tachycardic. She was attended by the A&E houseman, after doing all the physical examinations, getting the appropriate history, she was given the diagnosis of acute pelvic inflammatory disease, and was admitted to our own gynae ward.
The Gynae HO on call saw the case, and think that the presentation didn’t really fit in with the findings and history, and decided to consult his/her MO for advice, which was then followed by a better physical examination by the O&G MO, noting striae gravidarum n etc.
Basically you know what it is about - the girl is pregnant, or was pregnant.
An examination of her perineal area found bleeding lacerations/tears, which resulted in a lot of blood loss, contributing to her current presentation.
The question comes, where is the baby?
She denied all the while saying that she is not sexually active. Case was hand over to police. Investigations later made her admit to the crime she did. She gave birth to her baby IN the A&E department’s toilet, and wrapped the baby up with old newspaper and dumped the baby in the dustbin.
That must have been 1 day after the onset of the incident. A search was started to look for the baby but he/she was not found. 2 days later 2 male cleaners found the baby in a rubbish dump near the old labour room (not far from the place I stay). On inspection, it was cord around the neck of the baby x 4.
Post mortem kinda proved that the baby was alive when born.
So, did she practically gave birth to the baby in the toilet, then strangle the baby with his/her own cord? Cord around neck 4 times during delivery is seriously rare by nature.
And for me who is currently in the O&G department, seeing people happily give birth and cherish their babies, I can’t think of any sense that someone could actually do such a thing.
This whole story is like a crime tale, and the murderer happens to be the mother herself.
And during the talk, lots and lots of slides were there. So many dump case - river, toilet bowl, road side, throw out from the car window and etc etc. Such dirty places to celebrate start of life. Such stupid idea to kill.
Can’t help but to cherish that no matter how much parents can be protective, at least they gave us home, they gave us a proper chance to live. And not like those who choose to give up because of their own convenience and problem.
It is definitely a blessing to be 26 and healthy and moving :)
